Can you manage shared mailboxes and distribution groups?

Yes. Shared mailboxes, groups, aliases and similar Exchange Online administration are normal parts of Microsoft 365 management.

Is Microsoft 365 backup necessary?

That depends on the organization's recovery requirements. Microsoft provides platform resilience and retention features, but an independent backup can provide additional recovery options for deleted, changed or compromised data.
